Warranty/Disclaimer: I authorize the repair work on this invoice to be done with necessary material and grant Carmine's Import Service ("Carmine") and employee's permission to operate my vehicle on streets, highways or elsewhere for the purpose of testing and inspection. An express mechanics lien is hereby acknowledged on the vehicle to secure charges for repairs. If my repairs exceeded $350.00, I should have been given and have signed a repair order or estimate and know that my Limited Nationwide Warranty on parts and labor for most repairs is the earlier of 3 Years or 36,000 miles, whichever occurs first which began October, 2013 so long as my vehicle is serviced in accordance with the Carmine maintenance schedule or approved by management. Failure to follow the service schedule may void any Warranty expressed in this document. A manufacturer or dealership cannot void a warranty because your service was performed at an independent repair facility nor can they void your warranty if parts were not purchased from the manufacturer per the Magnuson Moss Act of 1975. An original equipment manufacturer (OEM) or original equipment supplier parts are essentially the same because they're built by the same manufacturer and are built to the exact same specifications as the vehicle's designer outlined. Here at Carmine's Import Service we are committed to providing you with the highest quality parts for your vehicle. Ask about our 4 year, 50,000 mile warranty when you follow our comprehensive maintenance schedule without missing one service. 4 year, 50,000 mile warranty effective June 26th, 2023. There is no reimbursement for repairs made at another shop due to a failure part installed by Carmine. Towing is NOT included for any failure to a repair we have made outside a 25 mile radius of Carmine. By signing the front, I acknowledge receiving the brochure and accept full responsibility for any service and repairs my vehicle may need. Customer also understands that while their vehicle is in for service or repair, Carmine is not responsible for any acts of God beyond our control including but not limited to items breaking during your visit due to old age, sudden warning lights during a test drive, flat tires, electrical malfunction, engine malfunction that is not Carmine's fault. However, Carmine backs up the quality of work and assures the customer that their vehicle will be serviced to the highest quality of standards. No warranty is provided on customer supplied parts if they are considered for installation. No warranty is provided on tires other than the warranty of the tire manufacturer which requires proper tire rotations and documentation which can be found on the manufacturers' website or provided by Carmine upon request. The tire suppliers are not responsible for tire wear from suspension issues, incorrect wheel alignment or failure to rotate tires or check air pressure. No warranty applies on wear/service items, used parts (unless expressed in writing), or failure on any part due to customer abuse, lack of maintenance, or neglect even if under the warranty period as determined by Carmine. Brake pad and rotor squeak/squeal is only covered for the first 90 days on non-performance models. No warranty is provided on transmission failure after a transmission service. There is also no warranty on performance parts or track car repairs. Customer responsible for any balance that a 3rd party warranty doesn't cover. Engine or other part failure or other failures resulting from pre-existing conditions or customer negligence such as engine oil sludge from extended oil services or continuing to drive vehicles with warning lights or low fluids are not covered. Any fuel or exhaust system component installed requires the use of a Carmine's approved ethanol fighting agent every 5k miles to maintain the warranty on that part. Warranty on commercial vehicles is limited to 30 days. Vehicle owner and/or client is responsible for knowing their vehicle is still under the manufacturer's warranty or extended warranty issued by the manufacturer. Carmine's is not responsible for voiding your manufacturer warranty due to the clients negligence or lack of knowledge of any outstanding warranties. While we are not in the business of locating the information for your factory warranty, we will notify you if we are aware of any warranties that exist on the vehicle during our diagnosis or repairs if we are informed. Warranty repairs of any kind must be performed in our shop & cannot exceed the original cost of the repair unless approved by and utilizing the Nationwide Limited Warranty through the 3rd party provider. Parts must also be available in order to issue the warranty. Warranty on all vehicles is subject to parts availability. If parts are no longer available there will be no warranty. Some vehicles prior to 1996 may not be eligible for the full warranty. Carmine's Import Service is not responsible for failure of parts on your vehicle due to age or damage from a repair not performed at our shop or any acts of god beyond our control. New Engine/Transmissions are only covered up to 2 years or 24,000 miles and proof of service history must be produced if not performed at our shop. Internal engine repairs, cylinder head gaskets, timing cover gaskets may not carry our Nationwide Warranty and may be only eligible for a 1 year, 12k mile warranty. Control module failure during programming not covered and is the responsibility of the customer to purchase a new module. Ethanol treatments must be purchased from us and installed every 5k miles to validate ANY warranties on fuel system components. All warranties are non-transferable, and a copy of this invoice is required for warranty service. Contact our staff for details. By paying for this invoice, you accept the terms and conditions stated above. If you have a 3rd party warranty, you understand that the company providing that service to you may not cover 100% of the total invoice and you will be responsible for the balance before picking your vehicle up. If your car is in for service and it breaks in our possession from something we did NOT cause, it is NOT our responsibility. It is your vehicle, your responsibility as an adult, and you accept the reality and logic of these terms.

Tesla Warranty: 6 month/6,000 mile LIMITED warranty on parts ONLY. Labor is NOT included. ZERO warranty on electrical parts and batteries. If you approve ANY repair order on a Tesla, you own the parts regardless if you back out before the job is completed and MUST pay for them in full. Note: Our 3 year warranty does not apply to Tesla vehicles.
ADAS Disclaimer: By reading this statement and accepting it, you choose to understand it. Advanced Driver Assistance Systems (ADAS) are a complex topic that leading 'experts' AND manufacturers cannot even explain. You, the driver and client, accept full responsibility for your car's driver assistance systems. You understand that these very new systems are in place ONLY to aid the driver and attempt to reduce accidents while the driver is in full control of their vehicle at all times. While your car should not drive itself into random objects so long as you are paying attention, you still agree to accept this responsibility as a LICENSED driver and a citizen of your community and pledge to not pass on your negligence to someone else because of something you think your car did to you. If your car hurts someone or crashes it is the fault of only yourself or another driver on the road that was in your path. Not the fault of the vehicle necessarily nor the vehicle manufacturer.
Arbitration Agreement: Any dispute, controversy or claim arising out of or relating in any way to the transaction you are having with Carmine's Import Service including without limitation any dispute concerning the construction, validity, interpretation, enforceability or breach of the repair order, shall be exclusively resolved by binding arbitration upon a Party's submission of the dispute to arbitration. The demand for arbitration shall be made within a reasonable time after the claim, dispute or other matter in question has arisen, and in no event shall it be made after 90 days from when the aggrieved party knew or should have known of the controversy, claim, dispute or breach.
Payment options: Cash, Cashier's Check Only, Bosch Credit, CarCareOne Credit, Visa, Mastercard, American Express, and Discover are all accepted forms of payment. If payment is not made within 3 days after notice by electronic or phone communication a $50.00 late fee will be added to invoice, after 5 days a late penalty charge of 3.00% of all amounts billed excluding local taxes will be assessed unless prior arrangements have been made with management. If the customer has not paid within 7 days of completion of the vehicle, Carmine reserves the right to file for a lien of said vehicle. Customers will be responsible for any fees such as interest and storage charges associated with violating the payment policy. Any vehicle which remains unclaimed after 30 days may be sold by Carmine within accordance of state law. We also reserve the right after 5 days to charge $30.00 per day in storage fees for each day the car is on our property after the completion of work and notification to the customer that work has been completed.*****(More in detail if you scroll down about your car being your responsibility and a detailed payment policy)***** Full payment may not be due before repairs BUT deposits WILL be required before service or repairs begin.
New Tire Registration: If you purchased new tires from Carmine's ?, you should register them by visiting https:/www.ustires.org/register-tires and choosing the brand of tire you purchased. The information you need can be found on your invoice. You are looking for the tire DOT numbers. You would register the number of tires you purchased. If you purchased four tires, you should have four DOT numbers on your repair order. If you do not, please contact our service department. An example of where these numbers are located is in the picture below. Carmine's Import Service ? is NOT responsible for sending you tire recall information in the event you forget to register your tire purchase. We suggest you do this immediately after purchase.
Abandoned Vehicles: A vehicle will be considered abandoned after 30 days of no communication from the owner whether or not the repair order is paid for. If the repair order is settled, the vehicle will be towed at the owners expense to a storage lot and the vehicle will then be dealt with by the storage lot. This is not a great experience for the owner. Not a nice process. If there is money owed on the vehicle you would refer to the paragraph above this one where we explain how we file a lien on your vehicle to settle your debt with us. This is also not a nice process and can be very costly too. The outcome is always bad for everyone involved. We don't want to do it and you don't want us to do it. All we want it to take care of your car and for you to have a great relationship with us. But in the event that is not possible for some reason, we have these rules in place so there is no chaos. By reading this, you agree to it when you replied approved to your repair order. Thank you for your understanding.
Recycled Parts Warranty/Disclaimer: If you have chosen a recycled part (used part) of any kind for your vehicle there is no warranty unless otherwise stated on your invoice. If there is a warranty stated on your invoice, it is a limited warranty and will cover only a total failure of the particular used part. The warranty will not cover any peripheral damage caused by the used part. The labor on that part is not covered. If you purchased a used engine or transmission, only an internal lubricated engine failure is covered under any limited warranty. Oil leaks, coolant leaks, accessory parts attached to that engine, ignition system components and other added components are not covered on a used engine installation. We strongly suggest you purchase any recommended items during the installation to avoid any possible failures after the job is completed. To obtain a full warranty on your purchase, a new or remanufactured engine or transmission would have to be purchased.
Except as otherwise specifically provided herein, Carmine's expressly disclaims any and all warranties or representations (whether express or implied, oral or written) with respect to its service or parts provided including any implied or express warranty of merchantability or fitness for a particular purpose, whether alleged to arise by law, by reason of custom or usage in the trade, or by course of dealing.
Carmine's is not responsible for loss or damage to the vehicle or any articles left in the vehicle in case of fire, theft, hail, wind, or any other causes beyond it's control;
Carmines personnel will/may turn off any photo or video capturing devices, such as dashboard cameras, the vehicle in preparation for service and your vehicle's dash cam/recorders will be enabled when you pick up your vehicle from this Service visit;
Labor charges are not based solely on actual service personnel's time but are aggregate prices for specific services or repairs, which may include flat rates based on industry manuals and vehicle condition;
Waste storage, Shop Necessities, and disposal fees are charged separately when applicable to specific services or repairs, and represent costs and profits to Carmine's which are calculated based on average annualized costs across Carmine's service facilities;
Items purchased over the counter or online directly from Carmine's may be returned within 15 days with a proof of purchase and must be in their original unopened and uninstalled condition with factory labeling attached and in factory packaging (if supplied);
Parts purchased directly from Carmine's over-the-counter, or online are covered under the Carmines Parts, Limited Warranty for a period of 12 months subject to the applicable terms, conditions and exclusions within this document.
All charges for repairs, including labor, parts and materials furnished, are due and payable simultaneously with the return of your vehicle or prior to return upon the expiration of three (3) working days after notice has been sent by Carmine's Import Service that the vehicle is ready;
If any repair, storage and other applicable fees remain unpaid for thirty (30) days after a request for payment, Carmine's may pursue collection and you will be responsible for paying all reasonable attorney's fees and costs for such collection;
If provided a loaner or rental vehicle, the vehicle must be returned within 24 hours of such notification or a daily usage rate of up to $150USD and applicable fees will be charged until the return of such loaner vehicle;
The repair work may not be completed prior to the date and time noted under Date/Time Promised and Carmine's may adjust the estimated completion date upon notification to you and is not responsible for any delays caused by the unavailability of parts or parts shipments;

Effective May 1st, 2019
Payment for all services is due upon completion of diagnostic assessment, and/or service/repair. Carmine's Import Service accepts cash and/or credit. If your account is past due, you may be required to provide a cashiers check or money order and not be permitted to use a credit card to settle your account at the discretion of management and you may be required to sign a delinquent past due account waiver.
Your account/repair order is considered past due 3 days after notice of completion unless prior arrangements have been made in writing via text or email and approved by management. Customers are notified by phone, text, or email or by all 3 that their account is due or past due. If payment in full has not been made after 3 days after notice, a $50.00 late fee will be added to the invoice. After 5 days a late penalty charge of 3.0% interest will be assessed and accrued daily of all amounts billed excluding sales tax. We also reserve the right after 5 days to charge $30.00 per day in storage fees for each day the car is on our property after the completion of work and notification to the customer that work has been completed. These fees including storage are non-negotiable and the customers responsibility unless special arrangements have been made prior. The customer's vehicle will not be returned to them until full payment is received including any fees.
If the customer has not paid after 7 days after service completion, our corporation reserves the right to begin to enforce our mechanics lien against the vehicle which was established when the vehicle was dropped off per the state of North Carolina. We will notify the State of North Carolina that the vehicle was left unpaid and the state will send you and any lien holders if applicable a letter that the vehicle is in jeopardy of being sold for the invoice amount including all storage fees and late fees. After 30 days have passed and all lien papers are filed and completed, the vehicle is therefore considered unclaimed and will be sold without hesitation in an attempt to settle your debt.
